We are open Monday through Friday (except Holidays) between 9:00 a.m. to 5:00 a.m. As a Specialist clinic, patients often need a referral from their General Practitioner, family physician or other physician in order to be covered by their insurance company or to be eligible for Medicare reimbursement.

When you come in for your appointment, please remember to bring the following:

• Insurance Card
• Driver's License
• Medicare card or the Veteran affairs card
• Have your insurance information
• Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.

To cancel an appointment:

Telephone the office during normal business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
